Every health system I talk to is bracing for the same storm: Medicaid cuts, shrinking margins, and tens, sometimes hundreds of millions in projected shortfalls. But here’s the part we don’t talk about enough: There’s a $𝟯𝟬𝗕 𝗺𝗮𝗿𝗸𝗲𝘁 growing right beside them that most systems earn $𝟬 from. It recently dawned on me that healthcare is driving on a two-lane highway while a brand-new third lane has opened and no one’s merging into it. 𝗟𝗮𝗻𝗲 𝟭: 𝗧𝗵𝗲 𝗴𝗼𝘃𝗲𝗿𝗻𝗺𝗲𝗻𝘁 𝗹𝗮𝗻𝗲 Medicare, Medicaid, regulations, reimbursement cuts — the traffic is thick, slow, and unpredictable. Some systems have 40–50% Medicaid populations. When the “one big beautiful bill” cuts hit, they’re staring at $50M–$500M shortfalls. This lane is congested. It’s not clearing anytime soon. 𝗟𝗮𝗻𝗲 𝟮: 𝗧𝗵𝗲 𝗰𝗼𝗺𝗺𝗲𝗿𝗰𝗶𝗮𝗹 𝗶𝗻𝘀𝘂𝗿𝗮𝗻𝗰𝗲 𝗹𝗮𝗻𝗲 Better reimbursement, but also tightening. Volume-dependent, operationally strained, margin-thin. You can only drive faster if you hire more, build more, and spend more. This lane is slowing too. 𝗟𝗮𝗻𝗲 𝟯: 𝗧𝗵𝗲 𝗽𝗿𝗲𝗺𝗶𝘂𝗺, 𝗼𝘂𝘁-𝗼𝗳-𝗽𝗼𝗰𝗸𝗲𝘁 𝗹𝗮𝗻𝗲, 𝘁𝗵𝗲 𝗼𝗻𝗲 𝗻𝗼 𝗼𝗻𝗲 𝗶𝘀 𝘂𝘀𝗶𝗻𝗴 It’s wide open. It’s growing. And it’s already carrying $𝟯𝟬𝗕 in spend: • Concierge care • Virtual-first access • Advanced diagnostics • Out-of-pocket memberships Patients are shifting lanes on their own, paying directly for speed, convenience, and clarity. Health systems are still in lane 1 and 2… wondering why traffic isn’t moving. Executives don’t see the third lane because it feels unfamiliar. It feels “nontraditional.” It feels like it sits outside the mission. But, the reality seems to be that the market didn’t shrink, it shifted lanes. That’s where a $99/month membership changes the game. It lets systems merge into the premium lane 𝘄𝗶𝘁𝗵𝗼𝘂𝘁: • abandoning insurance • cannibalizing existing clinics • or building new infrastructure It simply captures revenue that patients are 𝘢𝘭𝘳𝘦𝘢𝘥𝘺 spending elsewhere. If $30B is shifting into a new lane and health systems stay in the old ones, that isn’t fate. It’s a blindspot. The future advantage belongs to the systems that merge first. Because the third lane is already open and the traffic is moving without you. I started Amenities Health after serving as VP of Digital Health at Baylor. Health systems are about to make Kodak’s billion-dollar mistake. Medicaid cuts are coming. And the traditional playbook won’t save you. For years, leaders have tried to “bend the cost curve.” But here’s the hard truth: → Cost-cutting can’t offset billions in lost reimbursement. → Primary care, once a “loss leader,” is now a cost burden. → Incremental strategies won’t move the needle. This is a 𝗞𝗼𝗱𝗮𝗸 𝗺𝗼𝗺𝗲𝗻𝘁 for health systems. Remember Kodak? They invented the digital camera… then ignored it. They clung to the old model until it collapsed. That’s exactly where many systems are today. Two paths are left: 1. Reinvent your revenue model with premium, high-margin offerings. 2. Or keep squeezing pennies until the entire model breaks. And here’s the nuance most miss: premium care isn’t elitist. It’s the margin that sustains the mission. The future of healthcare won’t be saved by nostalgia. It will be saved by innovation. I started Amenities Health after serving as VP of Digital Health at Baylor. We’re now partnering with leading health systems to launch white-labeled, membership-based care programs that unlock $10M+ in 100% margin revenue and loyal patients. If membership or concierge-style digital care is on your radar, I’d love to get your feedback on what we’re building. Send me a direct message and let’s connect.

Here’s the trap most founders fall into when building an MVP 👇 They start with a feature wishlist: 1. Login 2. Dashboard 3. Notifications 4. Profile 5. Settings …and so on. But here’s the problem: Users don’t care about your feature list. They care about whether they can achieve their goal. That’s why you should start by mapping user journeys, not feature lists. 👉 “A patient books an appointment in under 3 clicks.” 👉 “A doctor uploads notes without retyping everything.” 👉 “A caregiver can track medication reminders without confusion.” Features are the tools. Journeys are the experience. When you design around journeys: ✅ You remove friction. ✅ You align with real outcomes. ✅ You stop wasting time building features no one uses. 💡 Next time you’re planning your MVP, ask: What’s the simplest path for my user to succeed? That’s your roadmap, not a bloated feature list. What do you focus on more, features or journeys? 👇 #WellnessWithAlchi

THE most common question we're asked on calls is "How is this free?" It was my first question too. (Actually, I think I said "I'm very suspicious!" to Amie Leighton 😆) Clinicians are so used to being sold a product (or become the product 😒) or asked to pay for a pricey subscription, that we've come to believe this is the only model that could possibly exist in this field. This is why I was so surprised when Saroosh Khan informed me that running an EHR, like Allia, for individual clinicians and small practices is relatively low-cost 😲 So in the words of our insanely talented Content Manager Areesha Hosmer, here is why Allia is free and always will be! 🔗 https://lnkd.in/g-yUZ5TG

Spent Monday touring what is likely the world’s leading ibogaine research lab at Columbia, with Professor Dalibor Sames (founder of Gilgamesh Pharmaceuticals), Leonard Pickard and David Lankri. Ibogaine is going to be a massive breakthrough for psychiatry and possibly medicine as a whole. Dr Nolan Williams’ conducted the first study to use neuroimaging to evaluate ibogaine treatment for traumatic brain injury and PTSD in special operations vets. The findings were incredible: an 88% reduction in PTSD symptoms, an 87% reduction in depression scores and marked improvements in cognitive function. No other drug has ever been able to alleviate the functional and neuropsychiatric symptoms of traumatic brain injury!! Gilgamesh is working on a cardio-safe ibogaine analog - which could remove the main barrier to wider use.

ISSX Europe 2026 is just around the corner, and I'm looking forward to spending a few days in Basel with colleagues, clients and fellow scientists from across the DMPK and ADME-Tox community. One of the things I enjoy most about ISSX is the opportunity to discuss the challenges that sit behind the data. Whether it's understanding unexpected metabolism, designing the right DDI strategy, improving candidate selection or navigating complex ADME properties, the most valuable conversations often happen outside of the presentation halls. If you're attending ISSX Europe this year and would like to discuss an ongoing programme or explore approaches to ADME-Tox and DMPK strategy, I'd be delighted to connect. I'll be attending alongside my colleague Laszlo Szilagyi Looking forward to seeing many familiar faces in Basel and meeting new ones as well! A few weeks ago, I had the privilege of being interviewed by Authority Magazine about the future of patient communication technology in healthcare. It was a great opportunity, but like most interviews, there were things left unsaid. Thoughts I self-edited. Questions I wish they’d have asked. Let's get into it together. The Question I Wish I’d Been Asked: "Why is AI held to a higher standard than humans?” And my answer: I've listened to thousands of calls. I've heard hospital operators hang up on bleeding post-op patients. I've documented 60% call abandonment rates. Yet when we propose an AI solution that answers 100% of calls with 95% accuracy, suddenly everyone requires perfection. The Question No One Asks: "What Happens When We Get This Right?". We already are. Houston Methodist handles 1.6 million calls annually with our virtual agents. Nebraska Medicine is automating 70% of its inbound calls. - Every call is answered immediately, in dozens of languages - Every appointment request is handled efficiently - Every prescription refill happens seamlessly - Every healthcare worker can focus on complex, meaningful patient interactions There is so much more to cover, but it really comes down to this: The technology works. The business case is proven. The patient outcomes are better. Read the rest of my thoughts here. And then let me know what you are waiting for. https://lnkd.in/ejJXwrec #patientengagement #patientaccess #aiinhealthcare

The Abridge announcement mostly covers it, so I’ll share a quick story: A couple months ago, I ordered a cardiac MRI for a patient. A month or so ago, I caught up on my inbox and found out that I needed to answer some questions and fill out some paperwork to justify the MRI. The questions were reasonable. I just wish someone could have had my back, and my patients back, and reminded me what to ask about and what clinical reasoning to document when I first thought of the MRI. And what if I then could have gotten that approval instanteously in the conversation… So here we are. Proud to announce Intelligence at the Point of Conversation. And privileged to announce Allegheny Health Network and Highmark Health - pioneers in all things prior auth innovation - as Partners. Confession time: I’m a control freak when it comes to HIM workflows (if you're in HIM, chances are you are too). Because when one form goes missing or lands in the wrong hands, it’s not just a mistake—it’s a risk. But most tools we use? They expect us to bend. Their workflows. Their logic. Their limitations. After 15 years in Health Information Management (HIM), I realized we needed smarter, simpler tools that understand the real work HIM teams do. That’s why I’m so excited about our ‘Conditions’ feature in Dexit’s Intelligent Document Processing (IDP). With user-defined ‘Conditions’, Dexit lets you build automated, self-reliant document workflows. It’s like finally being handed the steering wheel. You set the logic: ✅ “If document type is ‘Discharge Summary’ and it’s unsigned, flag it for clinical review.” ✅ “If Field A is blank, pause routing and send a task.” ✅ “If everything looks good, push it straight to storage—no human needed.” And Dexit takes it from there: ☑️ Executes predefined tasks when conditions are met ☑️ Flag exceptions in real-time for human review ☑️ Moves documents through the pipeline without bottlenecks No coding. No workarounds. Just your brain + your rules and automation that listens. For instance, imagine an incoming Release of Information (ROI) request. Under federal rules, you’ve got 30 days to respond under HIPAA. But if the patient resides in Washington? You’ve got 15 days. With Dexit’s ‘Conditions’, you can build this nuance right into your workflows. Here’s what that means for you: 👉 Automation that adapts to your workflows—not the other way around 👉 Built-in accuracy checks help catch risks before they escalate 👉 Human intervention only when needed = fewer errors and delays 👉 Relief from endless manual review = less burnout, more focus Bottom line: If your document workflows still rely on tribal knowledge and reactive cleanup, it’s time for a rethink. Smart, rules-based automation isn’t a luxury anymore—it’s a mission-critical strategy. Want to see how Dexit’s ‘Conditions’ feature works in action? Check out the quick demo video below! Are you one of them? I'm looking for the leaders who refuse to accept that achieving Epic Gold Stars and ensuring clinician satisfaction are mutually exclusive. After over 30 years as a physician and CMIO, I've learned a critical lesson: the most successful health systems don't treat clinician satisfaction as an obstacle to performance, they make it the engine that drives it. That's why I'm hosting a session with @314e Corporation on November 5th. This isn't just another webinar—it's a live walkthrough of the exact framework I’ve spent my career developing to unify these goals. You will leave with actionable strategies to transform your training program from a cost center into a high-ROI asset that drives both performance and retention.
