Wer schlecht schläft, sucht den Fehler meistens im Schlafzimmer. Ich suche ihn auch auf dem Teller und der Uhr. Neue Matratze, Blue-Light-Blocker , Melatonin- Tropfen – können eine gute Idee sein. Aber bei Hufgetrappel denken wir zumindest in Norddeutschland auch nicht als Erstes an Zebras. Wenn du dir um 21:00 Uhr noch eine schwere, kohlenhydratreiche Mahlzeit, die Tüte Chips oder die Tiefkühlpizza gönnst, ist dein Schlaf ruiniert, bevor du überhaupt die Augen schließt. Dein Körper ist erstmal damit beschäftigt zu verdauen statt zu regenerieren. Außerdem schießt dein Blutzuckerspiegel nach oben. Die Folge? Eine massive Insulinausschüttung. For me, it comes down to one thing: familiarity. Continuity and stable relationships aren’t just comforting, they’re protective. Familiar caregivers and steady routines provide the consistency needed to reduce stress, preserve dignity, and create a sense of safety. I’m often asked: Does it really matter if the same people come back day after day? From what I’ve seen, and what research confirms, the answer is yes. Trusted relationships make all the difference. The numbers underline the point: • 𝟭 𝗶𝗻 𝟭𝟮 𝗮𝗱𝘂𝗹𝘁𝘀 𝗼𝘃𝗲𝗿 𝟲𝟱 are now living with dementia. • 𝟴𝟱% 𝗽𝗿𝗲𝗳𝗲𝗿 𝘁𝗼 𝗿𝗲𝗺𝗮𝗶𝗻 𝗮𝘁 𝗵𝗼𝗺𝗲 𝗮𝘀 𝗹𝗼𝗻𝗴 𝗮𝘀 𝗽𝗼𝘀𝘀𝗶𝗯𝗹𝗲, surrounded by people and places they know. • Those who receive 𝗿𝗲𝗴𝘂𝗹𝗮𝗿 𝗶𝗻-𝗵𝗼𝗺𝗲 𝘀𝘂𝗽𝗽𝗼𝗿𝘁 𝗲𝘅𝗽𝗲𝗿𝗶𝗲𝗻𝗰𝗲 𝟯𝟬% 𝗳𝗲𝘄𝗲𝗿 𝗵𝗼𝘀𝗽𝗶𝘁𝗮𝗹𝗶𝘀𝗮𝘁𝗶𝗼𝗻𝘀, because familiar caregivers notice small changes before they become crises. But there’s another layer we can’t ignore: 𝘁𝗶𝗺𝗶𝗻𝗴. Studies show that agitation and confusion often peak in the 𝗹𝗮𝘁𝗲 𝗮𝗳𝘁𝗲𝗿𝗻𝗼𝗼𝗻 𝗮𝗻𝗱 𝗲𝗮𝗿𝗹𝘆 𝗲𝘃𝗲𝗻𝗶𝗻𝗴 (𝗮𝗿𝗼𝘂𝗻𝗱 𝟯–𝟴 𝗽𝗺). This “𝘀𝘂𝗻𝗱𝗼𝘄𝗻𝗶𝗻𝗴” period is when fragile routines are most at risk, and when unfamiliar faces can trigger the greatest distress. Continuity is critical in these hours. Without it, families might face higher medication errors, more readmissions, and greater anxiety as residents struggle to trust new people. With it, three powerful advantages emerge: • Familiarity that reduces disorientation, • Peace of mind for relatives • Tailored care that preserves dignity. At 𝗗𝗼𝘃𝗶𝗱𝗮, I’m proud that our 𝗖𝗶𝗿𝗰𝗹𝗲 𝗼𝗳 𝗖𝗮𝗿𝗲 safeguards this continuity. Each client is supported by a small, dedicated team that builds trust over time, making care proactive, not reactive. Because in dementia care, continuity isn’t a luxury. It’s the foundation for dignity, stability, and peace of mind.

If you know me, you’ll know I love a good story, and this one is about more than numbers. I'm excited to share the first published article towards my doctorate ("Validation of a Medication Adherence Risk Score (MARS) derived from transactional dispensary data as a possible predictor of mortality risk"). A huge thank you to my supervisors Natalie Schellack Christa Janse van Rensburg Audrey Jansen van Rensburg It's a topic that means a lot to me personally, as I believe in the power of using #evidence-backed concepts to influence practical change. The project consists of 3 articles touching on different elements of the topic. This paper (“Beyond prescriptions: chronic medication adherence predicts mortality risk in a large-scale cohort study") represents a #statistical analysis to show the ability of an #adherence-based score to predict #mortality risk. We developed a simple tool called the Medication Adherence Risk Score (MARS) built from #pharmacy transactional data to see how adherence to #chronic medication affects mortality risk. What we found: • For adults 25 to 55, a higher MARS score was associated with a higher actual-to-expected risk of death. • People with better adherence lived longer in time-to-event analyses. What to keep in mind is that this study used only private-sector data and ran through the COVID period, which can blur signals. However, even with those limits in mind, the message is clear. Encouraging better adherence is not just a nice idea - it has the ability to impact long-term outcomes (both #clinical and economic). It is a measurable risk factor that health systems, employers, and insurers can act on. I believe that this warrants future work on the topic, and a hard look at how we can incorporate adherence-based interventions into chronic #disease management - something that exists as a global #healthcare challenge. As organisations grow, complexity grows even faster – often slowing down organisations. One realisation I had recently: 𝗜𝗻 𝗴𝗿𝗼𝘄𝗶𝗻𝗴 𝗼𝗿𝗴𝗮𝗻𝗶𝘀𝗮𝘁𝗶𝗼𝗻𝘀, 𝗹𝗼𝗻𝗴-𝘁𝗲𝗿𝗺 𝘀𝘂𝗰𝗰𝗲𝘀𝘀 𝗶𝗻𝗰𝗿𝗲𝗮𝘀𝗶𝗻𝗴𝗹𝘆 𝗱𝗲𝗽𝗲𝗻𝗱𝘀 𝗼𝗻 𝗲𝗺𝗽𝗼𝘄𝗲𝗿𝗶𝗻𝗴 𝗹𝗲𝗮𝗱𝗲𝗿𝘀 𝗮𝗰𝗿𝗼𝘀𝘀 𝘁𝗵𝗲 𝘀𝘆𝘀𝘁𝗲𝗺 – 𝗻𝗼𝘁 𝗼𝗻 𝗰𝗼𝗻𝗰𝗲𝗻𝘁𝗿𝗮𝘁𝗶𝗻𝗴 𝗲𝘃𝗲𝗿𝘆 𝗱𝗲𝗰𝗶𝘀𝗶𝗼𝗻 𝗮𝘁 𝘁𝗵𝗲 𝘁𝗼𝗽. In highly innovative industries like ours, transformation is often not limited by strategy or scientific progress alone. It becomes limited by something else: organisational coherence. Every function has rational priorities. Commercial teams focus on growth and speed. Medical teams focus on scientific evidence and patient impact. Compliance focuses on risk. HR focuses on people and engagement. Communications focuses on transparency and orientation. Individually, all of these priorities make sense. 𝗕𝘂𝘁 𝗿𝗲𝗮𝗹 𝘁𝗿𝗮𝗻𝘀𝗳𝗼𝗿𝗺𝗮𝘁𝗶𝗼𝗻 𝗵𝗮𝗽𝗽𝗲𝗻𝘀 𝘄𝗵𝗲𝗻 𝗳𝘂𝗻𝗰𝘁𝗶𝗼𝗻𝘀 𝘀𝘁𝗼𝗽 𝗼𝗽𝘁𝗶𝗺𝗶𝘇𝗶𝗻𝗴 𝗹𝗼𝗰𝗮𝗹𝗹𝘆 𝗮𝗻𝗱 𝗮𝗹𝗶𝗴𝗻 𝗮𝗿𝗼𝘂𝗻𝗱 𝗲𝗻𝘁𝗲𝗿𝗽𝗿𝗶𝘀𝗲-𝘄𝗶𝗱𝗲 𝗽𝗿𝗶𝗼𝗿𝗶𝘁𝗶𝗲𝘀 – 𝗮𝗰𝗰𝗲𝗽𝘁𝗶𝗻𝗴 𝘁𝗿𝗮𝗱𝗲-𝗼𝗳𝗳𝘀 𝗶𝗻 𝘁𝗵𝗲𝗶𝗿 𝘃𝗲𝗿𝘆 𝗼𝘄𝗻 𝗱𝗲𝗽𝗮𝗿𝘁𝗺𝗲𝗻𝘁𝘀 𝗳𝗼𝗿 𝘁𝗵𝗲 𝘀𝗮𝗸𝗲 𝗼𝗳 𝗼𝘃𝗲𝗿𝗮𝗹𝗹 𝗴𝗼𝗮𝗹𝘀. The same is true for leadership. To me, this means focusing on building leadership capability across the organisation: encouraging leaders to make decisions, create clarity, empower their own teams, and resolve challenges at the right level – rather than relying on constant escalation. Strong organisations are built when leadership capability exists across the system – not only at the top. That requires a shared understanding of company goals and a willingness to make decisions where expertise and context sit closest to the business. Vanuit die vraag werkte ik recent een clinical case uit tijdens de initiële therapie (parodontale behandeling) met Perio•Aid en de inzet van VITIS Anti-stain. Deze casus laat zien: - Hoe mondhygiëne, patiëntgedrag (zelfzorg) en de gekozen mondverzorgingsroutine elkaar gedurende het behandeltraject beïnvloeden. - Dat veranderingen in één van deze factoren impact kunnen hebben op het behandelresultaat. - Dat behaalde resultaten altijd binnen de juiste context moeten worden geïnterpreteerd. Een belangrijke nuance is dat de patiënt gedurende de behandeling een goede mondhygiëne handhaafde. De verkleuringen bleven beperkt, waardoor geen uitspraken kunnen worden gedaan over het effect van VITIS Anti-stain als afzonderlijke factor. Binnen deze casus maakte VITIS Anti-stain deel uit van de mondverzorgingsroutine waarbij de patiënt weinig last had van verkleuringen en het product positief beoordeelde.
