Ask any doctor what inspired them to go into medicine. I guarantee not one of them will say, “for the paperwork.” And yet, physicians find themselves allocating nearly twice as many hours a week to admin tasks as they do to patient visits, resulting in less time for actual care and driving higher rates of burnout. The conversation around AI in healthcare brings up a lot of fears, including whether agents will replace clinical staff. I’d argue we should instead be focusing on how AI can serve as the liberation layer for clinicians – reducing admin burden and giving them more time for what actually moves the needle on outcomes: time spent with patients. Consider the work Trase is doing at Duke Health. Earlier this year, the team began deploying AI agents to help Duke address the challenge of mounting admin work, starting with a basic, but consequential, workflow: fax routing. Duke receives roughly 5,000 faxes each month. It’s work that its clinical staff previously spent almost 1,400 hours a month hand-sorting and routing. Before deploying a single agent, we worked closely with Duke to understand its team’s workflows, define each AI use case, set up proper tracking mechanisms, and establish KPIs for success. Within a month of deployment, fax triage ran 7.1 times faster, accuracy exceeding 98%, and Duke recovered nearly $300,000 in annual staff capacity, all without a reduction in headcount. Plus, because every agent is run on Origin – our operating system – we delivered these outcomes while also ensuring governance, auditability, role-based access, and human control. Fear of new technology is inevitable, and understandable. But when deployed thoughtfully and responsibly, it has the potential to, in the words of Manesh Patel, MD, Duke’s Chief of Cardiology and Trase’s first design partner, free clinicians “from the things that drive us crazy to do the things that make us more excited to do our job.” That’s the real promise of AI in healthcare: not replacing clinicians, but removing the burdens that keep them from the work that inspired them to go into medicine in the first place. Each week seems to bring another unsettling headline about AI agents running amok: • escaping sandboxes and hacking into external servers, breaking into restricted government files, deleting a company’s database and its backups, and most recently, submitting a false tip in an unsolved murder case to the Philadelphia Police Department. These are serious breaches with real-world consequences. But to me, the lesson shouldn’t be to stop building or deploying AI agents. It’s that governance must be built into the architecture from day one, not added on after deployment. That means AI agents should: • Access only the information required for their role • Escalate decision-making to humans when confidence falls below an established threshold • Create an auditable record of their actions • Remain subject to meaningful human oversight • Behave predictably – even as systems and conditions change Many of us working at Trase are all too familiar with these requirements, having built our careers in high-stakes environments where the consequences of getting AI wrong are enormous. That’s why we baked governance and compliance into our operating system: guaranteed HIPAA compliance for health systems, SRG-to-STIG automation for defense, and the list goes on. Electronic Health Records were supposed to improve care and liberate clinicians. While it’s true that they have digitized healthcare information, too often clinicians now find themselves spending more time entering and managing data than practicing medicine. Throughout my more than 40 years as a cardiologist and healthcare executive, I saw firsthand how administrative burdens like EHRs demoralized clinicians and syphoned their time away from patients. Today, agentic AI presents the first credible path to reversing that damage and creating the biggest opportunity to transform healthcare that many of us have ever seen. With healthcare facing serious structure pressures, the need is urgent: - Clinical data is growing exponentially, while our ability to process it has not - An aging public and the multimorbidity of the general population are placing increased demand on the system - Workforce shortages across all levels of staffing are contributing to burnout - Clinical trials are becoming more complex and expensive to operate Experts on agentic AI in healthcare have argued that AI can help restore the humanity of medicine by relieving clinicians of clerical and cognitive overload. I wholeheartedly agree. To be clear, that doesn’t mean replacing clinicians or their staff. Healthcare will continue to depend on human judgment, trust, empathy, and complex decision-making. But AI agents can absorb repetitive work so clinicians can practice at the top of their licenses and administrators can operate more effectively. That is what Trase is pursuing: starting with our work at Duke Health, where we are deploying agents designed to improve clinical workflows, streamline administrative tasks, and strengthen care coordination – with security and HIPAA compliance always at the forefront – so teams can focus on personalized clinical decision-making rather than cumbersome manual processes. Unlike EHRs, which contributed to clinicians’ burdens, I truly believe AI will deliver on its promise to return time to clinicians to deliver improved, more personalized care for their patients.
