Sound Long-Term Care Management (SLTCM), an accountable care organization (ACO) participating in the Medicare Shared Savings Program, delivered another year of significant Medicare savings. SLTCM President Michael Camacho recently sat down with Pearl S. from AJMC - The American Journal of Managed Care to discuss strategies for improving care and achieving meaningful savings. Better care isn’t a coincidence, and meaningful savings aren’t earned from maintaining the status quo. Sustainable, repeatable success comes from developing and supporting a proven, patient-centered strategy: 1. Data-driven decision support: Consolidating, aligning, and analyzing patient information empowers clinicians to act quickly and appropriately. 2. Strong care coordination: A dedicated multidisciplinary clinical team of experienced leaders and quality improvement nurses is essential to helping providers feel supported, which helps elevate care quality. 3. Intentional provider engagement: Transparency builds trust. Consistent communication fosters accountability and strengthens provider relationships. 4. Preventative care: Annual wellness visits, advanced care planning, and education go a long way to help keep patients healthy while limiting avoidable, high-cost interventions. 5. Telemedicine utilization: Harnessing purposeful technology can reduce burnout, improve after-hours support, and help reduce unnecessary hospitalizations. Sound’s ACO team is setting off once more, this time taking their road show out West. The team is taking some time to connect with one of our long-term care partners in Coeur d'Alene, ID. It’s a privilege to connect in person with the partners who drive our collective success. Every visit deepens the relationships that fuel stronger outcomes for at-risk patients in long-term care and assisted living patients. Stay tuned for a recap of our complete roadshow, featuring visits with our partners from coast to coast! Hard to spot. Deadly when missed. When it comes to sepsis, every minute matters. For long-term care nursing facilities, sepsis remains a huge risk to vulnerable residents. In this Skilled Nursing News article, see how telemedicine is changing the story. With 24/7, in-place care coverage, Sound telemedicine clinicians close nighttime and weekend care gaps—getting eyes on residents the moment early signs of sepsis appear. “Not only does [telemedicine] serve the health and well-being of residents, but it also helps ease the on-call load for clinicians,” says Carolyn Fogleman RN, Quality Improvement Nurse at Sound Physicians Telemedicine. While sepsis remains a formidable challenge, we invite you to read more about the technology that's reshaping the way we respond to it here: https://bit.ly/4pjlF5u
