There’s a lot of conversation about how to lower drug costs in this country. So let’s look at a policy that was supposed to do just that: the price-setting provisions in the Inflation Reduction Act. Three years later, it serves as a cautionary tale about the reality of price setting. The policy is undermining medical progress people are counting on. Since the IRA took effect, investment in early-stage small-molecule medicines has dropped nearly 70%, and post-approval cancer research has fallen more than 40%. That’s not a future any patient should be asked to accept. Meanwhile, the real drivers of patient costs remain untouched: middlemen who take half of every dollar spent on medicines. These entities include PBMs, insurers and 340B hospitals who don’t make medicines, and despite price setting, they continue to determine what patients pay and which treatments they can access. This is a uniquely American issue that drives up costs for patients. Patients deserve lower costs and continued breakthroughs. Reining in middlemen is how we deliver both. Dive deeper on the promises vs the reality of price setting in PhRMA’s latest blog:

Too many Americans are paying more for their medicines and facing delays because insurers and PBMs can add markups, deny coverage or require patients to try and fail other treatments before getting what their doctors prescribed. Direct purchase programs offer a better option. They allow patients to buy directly from manufacturers, with transparent pricing that can reduce costs and help people access their medicines more quickly. And starting in January 2026, AmericasMedicines.com will serve as a trusted site where patients, caregivers and employers can explore available direct purchase programs and patient support resources. The biopharmaceutical industry is doing our part. Now, policymakers must fix the system that lets middlemen drive up costs and delay access to care.
