ANCA-associated vasculitis is a rare autoimmune disease that can rapidly damage the kidneys, lungs, and other organs. Early recognition and prompt treatment are essential. In this episode of Kidney Commute: Pitstop, host Dr. Osama El Shamy is joined by rheumatologist Dr. Anisha Dua, nephrologist Dr. Duvuru Geetha, and kidney transplant recipient Melissa Rubio. Together they explore common signs and clinical presentations, diagnostic challenges, the role of ANCA testing and kidney biopsy, and why timely treatment initiation is critical to preserving organ function. Melissa shares her inspiring journey from diagnosis at age 17 through dialysis, kidney transplantation, and recovery. Her story highlights the importance of patient education, family support, and multidisciplinary care. Whether you are a member of the nephrology or rheumatology care team, this episode provides practical insights to help recognize ANCA-associated vasculitis earlier, support timely diagnosis, and improve patient outcomes.

Complement 3 glomerulopathy (C3G) and immune complex membranoproliferative glomerulonephritis (IC-MPGN) are conditions that lead to eventual progression to kidney failure for most people impacted by them. The landscape for C3G and IC-MPGN has undergone significant change. This session will discuss the latest insights on the pathophysiology of C3G and IC-MPGN and the role complement dysregulation. Similarities and differences between these two conditions will be discussed. Recognition, diagnosis and treatment strategies will be addressed. An update on the application of complement inhibition to manage C3G and IC-MPGN will also be included. A individual with C3G will provide a perspective on working with the healthcare team to manage care.

Cardiovascular–kidney–metabolic (CKM) syndrome drives substantial residual risk for cardiovascular events and kidney disease progression despite guideline-directed medical therapy (GDMT). This session provides a practical, case-based overview of applying current guidelines and emerging evidence to optimize CKM care using a multimodal, team-based approach. Faculty will highlight key guideline recommendations and recent clinical trial data supporting early, comprehensive GDMT, with a focus on integrating aldosterone-targeted therapies, including nonsteroidal mineralocorticoid receptor antagonists and investigational aldosterone synthase inhibitors, to address persistent cardiokidney risk. Attendees will also learn strategies for care coordination, improving adherence, and overcoming system-level barriers. Through clinical cases and practical frameworks, participants will gain actionable approaches to implementing evolving therapies in real-world CKM care.
