Dr. Anahita Dua’s Hot Take: Plain old balloon angioplasty (POBA) is the most OVERRATED device in tibial intervention and Deep Venous Arterialization (DVA) is the most UNDERRATED. In this clip from Episode 570 of the Backtable Podcast, Dr. Dua believes POBA’s time in the spotlight is over with an assortment of effective devices on the market today. While our host, Dr. Sabeen Dhand, and Dr. Dua agree: We will likely see DVA happen sooner for at-risk patients. This weekend on BackTable: Sabeen Dhand talks with vascular surgeon Anahita Dua about tibial laser atherectomy, its role in limb salvage, and the upcoming randomized control trial AMBITION BTK comparing it to angioplasty alone. "What people want to know is where is the space where atherectomy does help? And that's what this (AMBITION BTK) trial is all about, giving you that answer as a community specifically for laser atherectomy." - Dr. Anahita Dua Watch or listen to the full episode here: https://lnkd.in/eui3uu53 This educational content is supported by AngioDynamics. Single-session thrombectomy has been reshaping how acute arterial occlusions are treated. With fewer ICU beds for overnight lysis and less OR time for second looks, Dr. Khanjan Nagarsheth describes why one-stage percutaneous treatment has become his default, while Dr. Shang Loh makes the case that lysis and open embolectomy still have a place. Dr. Sabeen Dhand then asks both vascular surgeons which aspiration, mechanical, and combination devices are on their shelves today. Radiologists now perform a growing number of bone marrow biopsies, and many of these referrals come from hematology teams who once did the procedure at the bedside. Dr. Mohammad Samim of NYU Langone Health and Dr. Christopher Beck walk through what makes the procedure efficient and well tolerated: who actually needs imaging guidance, platelet thresholds, a periosteal block that lets most patients skip sedation, trajectory planning on the scout, and when a coaxial system earns its place. They also cover variable speed powered drivers in soft and sclerotic bone, what to do with a dry tap, and why a slow, steady pull makes the aspirate hurt less.
